Eleanor T. Kozlik, 91 formerly of Chicago, passed away on Saturday, December 26, 2015 at Autumn Leaves in Crystal Lake following a lengthy illness.
She was born January 9, 1924 in Chicago, the daughter of Nicholas and Lucille (Maniero) Scavallo. She was united in marriage to Ben R. Kozlik on February 12, 1950. The couple made their home in Chicago.
She is survived by her sons, James (Marian) Kozlik of Emigrant, Montana, Ken (Linda) Kozlik of Algonquin, her daughter, Nancy M. (Terry) Moan of Tucson Arizona, her grandchildren, Davin (Patricia), Michael, Christabelle and Heather (Jason), her great grandchildren, Stewart, Savannah, Emily & Zachary and her sister, Lorraine D’Agostino of Chicago.
A memorial gathering will be held from 5:00 – 7:00 p.m. on Wednesday, December 30th at the DeFiore-Jorgensen Funeral & Cremation Service, Huntley. In lieu of flowers, the family requests memorials to the Alzheimer’s Association.
